GLP-1 receptor agonists — semaglutide (Ozempic, Wegovy), tirzepatide (Mounjaro, Zepbound) and liraglutide (Saxenda) — have changed what is achievable in obesity medicine. They are also, in Peru as elsewhere, widely sold to people who were never assessed for them.
That is the part worth being direct about. The drug is the easy component. Deciding whether you are a candidate, escalating the dose at a pace your body tolerates, protecting your muscle mass while you lose fat, recognising the side effects that matter versus the ones that pass, and planning what happens when you stop — that is the treatment. Without it, most people either abandon the medication within three months or regain what they lost.
Who is a candidate
Broadly, these medicines are indicated for:
- BMI of 30 or above, or
- BMI of 27 or above with a weight-related condition — type 2 diabetes, prediabetes, hypertension, dyslipidaemia, obstructive sleep apnoea, fatty liver disease or PCOS
They are not appropriate for cosmetic weight loss at a normal BMI, and they are contraindicated in pregnancy, in personal or family history of medullary thyroid carcinoma or MEN2, and in active pancreatitis.

Buying these medicines in Peru: read this first
Semaglutide and tirzepatide are available in Peru through licensed pharmacies, and they are also sold through channels that are neither licensed nor safe. Counterfeit pens are a documented problem in the region, and a counterfeit pen is not a weaker version of the real thing — it is an unknown substance you are injecting weekly.
Three rules that will keep you out of trouble:
- Buy from a licensed pharmacy against a prescription. If a seller does not ask for one, that tells you what you need to know.
- Check that the product is DIGEMID-registered. DIGEMID is Peru's medicines regulator; registration can be verified.
- Refuse compounded or repackaged vials. Legitimate GLP-1 products come in sealed manufacturer pens.

How the treatment is run
- Candidacy assessment — full history, weight trajectory, previous attempts, and the conditions that change the calculation.
- Baseline laboratory work — metabolic panel, HbA1c, lipids, thyroid and liver function.
- Choosing the molecule — semaglutide and tirzepatide behave differently, and the right choice depends on your starting point, your comorbidities and, realistically, on cost and availability.
- Gradual dose escalation — the schedule exists to keep side effects tolerable. Rushing it is the single most common reason patients quit.
- Protecting lean mass — adequate protein and resistance training are not optional extras here. Weight lost without them includes muscle you will want back.
- An exit plan — how the medication is tapered, and what maintains the result afterwards.
The side effects, honestly
Most are gastrointestinal and most improve: nausea, constipation, reflux, a feeling of fullness that arrives faster than you expect. They are dose-related, which is precisely why escalation is gradual.
Less common but genuinely important: pancreatitis, gallbladder disease, and significant dehydration from persistent vomiting. You will get clear instructions on what to do and when to call — before you need them, not after.

- Weight that returns after every attempt at losing it
- Prediabetes or type 2 diabetes alongside excess weight
- You are already using a GLP-1 and want it properly supervised

Can I get a prescription at the first consultation?
Often, yes — provided the assessment supports it and there is no contraindication. What will not happen is a prescription without an assessment. If baseline laboratory work is needed before starting, I will tell you plainly why, and it is usually a matter of days rather than weeks.
How much does GLP-1 treatment cost in Peru?
Monthly medication cost depends on the molecule, the dose you reach and the pharmacy, and it changes often enough that any figure quoted here would be out of date. It is generally lower than US list prices and higher than most patients expect. We go through realistic monthly cost at the assessment, because a plan you cannot sustain financially is not a plan.
Will I regain the weight if I stop?
Some regain is common, and pretending otherwise would be dishonest. How much depends heavily on what was built during treatment — muscle mass, eating patterns, activity. That is why the exit plan is part of the treatment from the beginning rather than an afterthought at the end.
